BOD Reward Bug [Can't reproduce]

According to the chart, a 20 exceptional gold SBOD has a 100% chance of giving a colored forge.  Out of 3 I've done, I've received 2 +5 hammers and one colored forge. 

I don't recall exactly the first SBOD that gave me the hammer, but the last SBOD was a 20 exceptional gold chainmail coif.

I've tried to reproduce this numerous times but every single 20 gold exceptional smith bod I've turned in has yielded a colored forge as the result. The only way I was able to get a +5 hammer as a result was by reducing the quality to non-exceptional.

I tried 40+ times with various different item craft types just to be sure, even though the script does not check the item type on small bods (just to make sure I wasn't missing anything). So I don't have a clue how this could have happened unless you made a mistake about whether or not exceptional was required.
