I'm somewhat aware of this issue as it has happened to me while playing CTF before. I skimmed over the code at the time but couldn't see where it was occurring. I'll take another look now and see if I can figure it out.
What's weird about this is the message "The Red flag has been returned to base!" never occurs in your journal. Can you confirm you didn't accidentally edit that out?
Edit: It looks like this might happen when someone on your team idles out of the game.
